The Director, Health Solutions – Chronic Condition Management (Director, Population Health Strategy) owns the strategy, development, and performance of a national portfolio of products, programs, benefits, partnerships, and care models that improve outcomes for Medicaid members with chronic and complex conditions. Using population data, clinical evidence, member needs, and market priorities, the Director, Health Solutions – Chronic Condition Management (Director, Population Health Strategy) leads a team that develops, evaluates, and improves condition management solutions.

The Director, Health Solutions – Chronic Condition Management (Director, Population Health Strategy) sets investment priorities and measures of success and works across teams to scale solutions responsive to state and member needs. You will own portfolio strategy and performance, while operational teams implement and deliver the solutions.
